Congratulations! Your employees have nominated your company as a great place to work. In order to apply for this year’s "Best Place to Work" award, we require that you provide more information about your organization and its HR practices.

To enter the competition, you must complete and submit all portions of this form no later than July 25, 2008, by 5:00p.m. ONLY ELECTRONIC SUBMISSIONS WILL BE CONSIDERED. Be sure your application is submitted by the deadline. NO LATE SUBMISSIONS WILL BE CONSIDERED.

We will be pleased to answer questions and/or assist you with the application process. Please feel free to contact our Best Places to Work Committee Chair Denyse Wortham, SNHRA Executive Administrator at denysew@snhra.org.

Are you the individual responsible for filling out the awards entry form on your companies Human Resource practices?  If not, please go back and submit the Nomination form first. We will contact your HR Director for more information.

Please be sure to answer all questions as completely as possible and provide any additional information you feel might help you earn a spot as a finalist in the program.

Suggestion: Feel free to print out this page before filling it out to gather all of the answers before coming back to submit the application. Write your Part III - HR Practices in Word, then copy and paste your text into this form before submitting.
